Student Eligibility and Forms
Parental and Medical Consent is Required
To be eligible for participation ALL students must provide a completed PSAL Interscholastic Athletics Parental Consent Form and Interscholastic Sports Medical Form to the HSEF Athletic Director, Mr. Cannon in room 702 prior to a PSAL Sport activity, including try-outs.
These forms must be completed by a parent and physician.
